The Incident Report form must be completed by the affected party. Once this form is completed, you must give it to the Regional Commissioner, Safety Director, Area Director or Tournament Director for their signature. They will forward to Risk Management for processing.

The Participation Release Form is completed by the parent or guardian of the child that is returning back to play after an illness or an emergency.
